What We Do
Senior Counseling Group, LLC contracts with homes to be their psychological service provider. Each contracted home is assigned a provider who is either a Licensed Psychologist, Licensed Clinical Social Worker, Licensed Professional Counselor or Clinical Nurse Specialist who visits residents on a weekly basis.
Together, the assigned licensed provider and resident develop a plan to reach their goals in psychotherapy. The assigned provider meets on a regular basis with the social services team to inform on resident’s progress and problem solve issues that may arise in psychotherapy.
Psychotherapy Program
Our program provides assessments and individual psychotherapy for residents experiencing some of the following:
- Grief and Loss Issues
- Sadness
- Anxiety
- Depression
- Isolation
- Behavorial Changes
- Difficult Transitions
- Changes in Mental Status
- Poor Appetite and Weight Loss
- Loss of Personal Status
Initial Diagnostic Assessment
Resident is assessed for present and past history including any long standing mental health diagnosis. A care and treatment plan is designed with a focus on objective, measurable and outcome based goals.
Individual Psychotherapy
One on one weekly therapy sessions to improve quality of life, reduce symptoms of anxiety and depression, and to help adjust to (particularly in nursing home patients) loss of status and independence. A collaborative, therapeutic relationship is established with resident aimed at achieving the outcome based goals established in treatment plan.
Treatment Plan Reviews
Quarterly reviews to access treatment goals, possibilities of discharge, or continued services if warranted.
Patient Referrals
Types of patient referrals:
- Symptoms of depression, schizophrenia, anxiety
- Challenges to compliance with nursing home care plan
- Behavioral Challenges
- Alcohol and substance abuse
Providers
To become a contracted provider for Senior Counseling Group, you must be licensed in the state where services are provided. Each contracted provider is provided administrative services, training, clinical coordination between providers and assigned home(s), monitoring of Center for Medicare services regulations, referral processing, billing and collections.
